C.A.R.E.S., Inc. provides assessment and case management services for veterans with disabilities through a contract with the Togus Veteran’s Administration (VA) Center, Chapter 31, Independent Living Program.

The purpose of the assessment is to identify barriers to independent living in home/community and to make recommendations how to remove or modify these barriers.

C.A.R.E.S., Inc. staff coordinates and assists veterans in obtaining the services identified in the Independent Living Plan (ILP). Examples of case management activities include:

  • Identifying potential contractors and vendors.
  • Securing bids and estimates for services identified in the ILP.
  • Coordinating the inspection and invoicing of completed projects.
  • Coordinating with other VA program’s such as Prosthetics, Home Improvements and Structural Alterations (HISA) and Specially Adapted Housing (SAH), when appropriate.
  • Conducting regular telephone and face-to-face contact with veterans in the Independent Living Program.
  • Providing thorough documentation of all case activities.
  • Submitting monthly reports to the VA for all veterans we are assisting.
