C.A.R.E.S., Inc. provides case management services for veterans with disabilities through a contract with the Togus Veteran’s Administration (VA) Center, Chapter 31, Independent Living Program. C.A.R.E.S.’s staff coordinates and assists veterans in obtaining the services identified in the Independent Living Plan (ILP). Examples of case management activities include:

Identifying potential contractors and vendors;
Securing bids and estimates for services identified in the ILP;
Coordinating the inspection and invoicing of completed projects;
Coordinating with other VA program’s such as Prosthetics, HISA and SAH, when appropriate;
Conducting regular telephone and face-to-face contact with veterans in the IL Program;
Providing thorough documentation of all case activities; and
Submitting monthly reports to the V.A. for all veterans we are assisting.
